Academic Standards Committee Petition Form

Only students in good financial and academic standing are eligible to petition unless they are instructed by an ACHS staff member to complete this form.

Petitions are reviewed by the appropriate ACHS committee, and students are notified by email of the Committee’s decision. Committees typically meet once per week to discuss petitions. Responses can be expected by email within two weeks of submission.
